Who We Are

Welcome to Health Choice Community Partners

Health Choice Community Partners, LLC., (HCCP) was formed as an accountable care organization (ACO) in 2022 with the mission of improving the health status of our communities through the provision of affordable, evidenced based and community-based care. HCCP aims to promote and support quality medical homes through the re-investment of savings back into the community and through the meaningful use of health information technology and data analytics.

 

HCCP was formed by community health centers and Health Choice Care (HCC) to develop a provider-sponsored, community health center-based organization capable of providing quality care to our communities while adapting to changes in the health care environment. HCCP focuses on achieving the quadruple aim of improving the health of populations, improving the clinician and patients experience of care, and reducing per capita health care costs.

 

HCCP is jointly owned by seven community health centers located throughout Arkansas, Florida, Kentucky, Mississippi, Illinois, Texas, Rhode Island, and Health Choice Care (HCC).

About ACOs

The Centers for Medicare & Medicaid Services (CMS) established the Medicare Shared Savings Program (Shared Savings Program) to facilitate coordination and cooperation among providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries and reduce unnecessary costs.

 

The Shared Savings Program is designed to improve beneficiary outcomes and increase value of care by:

  • Promoting accountability for the care of Medicare FFS beneficiaries

  • Requiring coordinated care for all services provided under Medicare FFS

  • Encouraging investment in infrastructure and redesigned care processes

 

Eligible providers, hospitals, and suppliers may participate in the Shared Savings Program by creating or participating in an Accountable Care Organization (ACO). An ACO is a group of physicians, hospitals and other providers who voluntarily join together to assume responsibility for the quality and total cost of care for their patients. ​ACO providers work together to improve health care quality, coordinate patient care, and reduce costs.​ ACOs are eligible to receive a portion of savings achieved by reducing costs and meeting quality requirements.
