(HCCP) is an Accountable Care Organization (ACO) formed by community health centers and Health Choice Care (HCC) to develop a provider-sponsored, Federally Qualified Health Center (FQHC) led organization capable of providing quality care to our communities while adapting to changes in the health care environment.
